Here is an example of a personal value proposition statement.:

My purpose is to leverage my (talents/gifts) creativity 
by contributing (actions/behaviours within my capacity) break through ideas and creative solutions 
that results in (growth and fulfillment) exciting projects for my team 
all while honouring my (values) of collaboration and integrity.
